Alchemy stones information

The recipe I use for creating alchemy stones is

Shining Powder x9, Blood of the Sinner x6, Clear Liquid Reagent x10, Pure Powdered Reagent x10, Powder of Time x7

Which I have to look up every time since it doesn't save it in the alchemy tools that I can find.

I've read that it can give you a dud stone even if you use the right recipe but it's never happened yet in the 5 or so times I've tried it. If you succeed it is random on getting a life, destruction, or protection stone.
